- Canadian Birth Certificate/Citizenship/PR Documents
- Copy of current Teaching Certificate
- Statement of Professional Standing*
- Copy of proof of change of name (if necessary)
- Proof of proficiency in English or French
Fees
- Application fee: $170(teachers educated in Ontario)
- Application fee: $392 (teachers educated outside Ontario, includes $140 application fee plus $222 evaluation fee)
- Evaluation fee: $222
- Annual fee: $150
